The first twin took non steroidal anabolics since he has an very even diffuse looking MBP. The second twin has patchy healthier areas of hair still remaining. When you get very even, diffuse looking hair loss it is a real indicator of supplement use since the hair loss is systemic and all the DHT prone hair is subject to it's effect.
This is also seen with medications that cause hair loss such as beta blockers or statins. Your hair transplant doctor with a good H&P will rule out the cause of such hair loss.
